How to Obtain a Quote

The United Way Toronto (UWT) offers a group insurance plan through Great-West Life Assurance Company. Life and long term disability coverage are mandatory requirements under the plan. All other benefits are optional by Agency.

If you are interested in obtaining a quotation on your benefit package, please provide the following information;

• Employee data - sex, date of birth, salary, position, coverage required (single, family or spousal exemption). Names are not required. Download Employee Data Sheet (in excel format).
• Claims experience over the last two years (if 25 or more employees are insured under your plan).
• Rates over the last two years.
• Outline of current benefit package (or copy of the benefit booklet).
• Number of employees on long term disability benefits, if any.
• Renewal Date.
• Letter requesting The Deacon Group obtain this quote on your behalf. Download sample here.
